Documentation is the part of the job nobody trains you for and everybody takes home. Most five-minute-note advice assumes you have a template-friendly EMR. Plenty of us don't.

This guide covers both. It's the structure I use to get a defensible note written before I leave the driveway: work off an occupation checklist, combine subjective, objective, and assessment instead of writing them three times, and keep the plan separate so it's easy to update.
