YOUR TIME IS VALUABLE

Write your SOAP notes in 5 minutes or less

Documentation is the part of the job nobody trains you for and everybody takes home. Most five-minute-note advice assumes you have a template-friendly EMR. Plenty of us don't.

This guide covers both. It's the structure I use to get a defensible note written before I leave the driveway: work off an occupation checklist, combine subjective, objective, and assessment instead of writing them three times, and keep the plan separate so it's easy to update.

Here's what you get:

  • The occupation checklist that does most of the work for you

  • How to combine S + O + A without losing anything a reviewer needs

  • Why the plan stays separate, and what belongs in it

  • Two versions: one for EMRs that support templates, one for the ones that fight you

The Time Saver SOAP Note Guide

If you are finding that you are spending more than 10 minutes on each note, this may be for you. After becoming a private practice owner and a vendor for my local regional center I knew I needed to make the most of my time, and doing notes is not what I wanted to spend most of my days doing.
